• THE FILE for authorization by REGISTRATION

1. Application;
2. Copies of the documents proving that the applicant is a legally established entity;
3. Technical documentation;
4. Copies of the authorizations or endorsements issued by other state bodies, required by law;
5. Other information considered necessary by the applicant in support of the application;
6. Proof of payment of the fee and tariff (Regulation on fees and tariffs).

· The technical documentation for registration shall specify, as the case may be:

· The type of source or radiological installation;
· The design variant;
· The components;
· The maximum parameters;
· The identification data;
· The year of manufacture;
· The supplier
· The quantity
· The date on which the product was obtained;
· The uses to which it is put;
· The location within the legal person;
· The manner in which the instructions for use are complied with;
· The necessary radiation protection measures;
· The verification and testing interval.

· The documentation shall be accompanied by a copy of the invoice or of another document proving the origin and, as the case may be, by a copy of the acceptance tests performed by the installer.
· The documentation shall contain the details of the responsible person, mentioning the practice permit, the general training and the training in the nuclear field.
