• The application
The application for authorization/approval shall contain at least the following information:
a) the identification details and registered office of the applicant and of the subunit, such as a subsidiary, branch or administrative division, through which the nuclear activities are carried out;
b) the nuclear activity intended to be authorized, or the types of packages or special form radioactive material, low dispersible radioactive material or means of containerization for which design approval is requested, or the radiation protection programme for special use vessels, or the calculation of the values for radionuclides not listed in Table 1 of NFTSMR for which approval is requested;
c) identification of the person who can legally bind the applicant, mentioning the position held and the means by which he or she can be contacted - telephone, fax, e-mail - and, where applicable, the name of the person empowered to represent the applicant in its dealings with CNCAN;
d) the postal address to which correspondence can be sent;
e) identification of the responsible person;
f) identification of a contact person for any specific technical issues that may arise in the authorization process and the means by which he or she can be contacted;
g) the information required by the laws in force to give it legal effect in relations with third parties and to engage the liability of the legally established entity (including the signature of the person empowered to represent it and the stamp).
