• The application
The application for registration or authorization shall contain at least the following information:
a) the identification data and registered office of the applicant and of the sub-unit, such as a subsidiary, branch or administrative division, through which the nuclear activities are carried out;
b) the nuclear practice or activity intended to be authorized, or the types of radiation sources, radiological installations, material or device used for protection against ionizing radiation, etc.;
c) the identification of the person who can legally commit the applicant's liability, stating their position and the means by which they can be contacted - telephone, fax, e-mail - and, where applicable, the name of the person authorized to represent the applicant in its dealings with the Commission;
d) the postal address to which correspondence may be sent;
e) the identification of the radiation protection expert, where applicable;
f) the identification of a contact person for any specific technical issues that may arise during the authorization process, and the means by which that person can be contacted;
g) the information required by the laws in force to give it legal standing in relations with third parties and to engage the liability of the legally established entity (including the signature of the person authorized to represent it and the stamp)
